Planning stroke rehabilitation at home with your care team

Begin with the discharge and rehabilitation plan

After a stroke, rehabilitation needs differ from person to person. Movement, communication, daily activities and emotional wellbeing may all need attention. Physiotherapy can be one part of a wider plan rather than a replacement for every service involved in recovery.

The NHS describes stroke recovery as individual and dependent on its effects and the rehabilitation plan. When arranging a home visit, keep the hospital’s instructions and follow-up information available so the assessment can be coordinated with existing advice.

Describe everyday difficulties clearly

Explain which activities currently require help: moving in bed, sitting, transfers or getting around the home, for example. Tell the booking team if another person must be present or if the patient has difficulty communicating. This helps organise the visit; it does not replace assessment.

Do not select exercises or equipment solely from an online checklist. Ask the treating professionals which activities are appropriate and what assistance is needed. Avoid trying a new transfer technique without suitable instruction.

Prepare for caregiver questions

  • Which activities need supervision or assistance?
  • What should the caregiver practise only after instruction?
  • How will fatigue and rest be considered?
  • Who should be contacted if function or symptoms change?
  • When will the rehabilitation goals be reviewed?

Arrange a realistic home-visit schedule

In Kolkata and surrounding areas, confirm the full address, lift access and time when a caregiver can attend if needed. Discuss appointment frequency after assessment. A package is an administrative arrangement, not a guarantee that recovery will follow a fixed timetable.

CareMotion’s stroke rehabilitation service can be discussed with the booking team, subject to suitability and availability. Continue planned medical follow-up and use the relevant emergency service for a suspected new stroke or other emergency; do not wait for a routine home-physiotherapy appointment.
