ACL Biology: Graft Remodeling and Early Joint Protection
The Anterior Cruciate Ligament (ACL) is the primary restraint against anterior tibial translation and knee rotational instability. Following surgical reconstruction (using hamstring, patellar tendon, or quadriceps autografts), the new graft undergoes a biological process called 'ligamentization'.
Between weeks 6 and 12 post-surgery, the graft undergoes structural revascularization and is biologically at its weakest mechanical state. Rehabilitation must strictly protect the graft from open-chain shear forces while rebuilding muscle support.
- Controlling acute postoperative hemarthrosis (joint swelling) with cold therapy and elevation.
- Restoring full passive terminal knee extension (0 degrees) identical to the uninjured leg.
- Overcoming arthrogenic muscle inhibition to re-engage the dormant vastus medialis.
Phased Exercise Progression from Crutches to Squats
Rehabilitation progresses systematically: Phase 1 emphasizes extension, patellar mobility, and quad sets. Phase 2 introduces closed-kinetic-chain exercises (mini-squats, leg presses, and stationary cycling) that generate joint compression rather than dangerous shear.
Progression to jogging and agility drills requires passing objective clinical criteria: zero knee joint effusion, at least 90% quadriceps limb symmetry index (LSI), and stable single-leg hop testing.
- Static quad sets, straight leg raises in an extension brace, and patellar glides.
- Closed-chain wall slides and double-to-single leg bridges to reactivate glutes.
- Stationary cycling without resistance once knee flexion reaches 100 degrees.
Home Physiotherapy and Functional Independence in Kolkata
Traveling to a clinic on crutches through Kolkata traffic carries slip and trip hazards. CareMotion brings post-ACL surgical rehabilitation into your home, ensuring safe step-by-step progress under direct clinical supervision.
Our sports physiotherapists guide gait retraining, stair negotiation, and neuromuscular balance drills in your living room.
- Avoid open-chain seated knee extension with heavy ankle weights between 0 and 45 degrees.
- Ensure walking crutches are fitted to wrist height with 20 degrees of elbow bend.
- Do not rush return to pivoting sports before 9 to 12 months post-surgery.
